Considering preseason expectations + how they started (i.e. taking 3 of 4 from the Doyers in LA/impressive fashion), I'd argue this was the worst season collapse in SD Sports' History.
Can anyone think of any other that comes close?

I'm make Lamet get surgery or trade him. And he'd file an immediate grievance.

Maybe they could DFA him … but I wouldn't go that far. He does have some serious issues to work through this winter. If he's not 110% by next April, there may need to be some reckoning. And who know, maybe there won't be a contract at that time - though in many cases (who knows if this even happens in baseball given their special antitrust status), if contracts in regular business situations go to arbitration, the previous contract is typically kept in place until there's a ruling.

Can anyone think of any other that comes close? Not really, the Padres were certainly the kings of suck when they were young, but there was no expectation back then that they'd be any good. Maybe the Chargers' 2003 season - the year of the big fire that took them to play a home game in Arizona because we used the stadium as an evacuation site. That was a bad fall all the way around. But the expectations on the Chargers that season were nowhere near what the expectations were for this year's Padres team. While the '03 Chargers featured Drew Brees, L.T., Antonio Gates, and some other noteworthy future Chargers' stars, they were just getting started back then. Brees and L.T. were both 24 that year, and Antonio was just 23. I'd have to agree that this year's Padres Suckage was pretty near epic proportions…

He has a torn UCL. He's not going to be 110%...ever. He'll go through arbitration just like every other player eligible.

Basically he's going to be a reliever for the rest of his career, because he's afraid of having surgery with the possibility that the surgery might not work out?

He's never been able to handle a starting workload without injury. Having a second Tommy John operation this close to his first surgery is a major issue.
